Misty Landor is the Senior Director of Care Management at Village MD in Atlanta, GA where she is responsible for the vision, strategy, leadership, planning and management of the development and operational oversight of cost-effective outpatient care management programs. As a nurse practitioner, she utilizes her advanced clinical skills to successfully educate registered nurses on holistic clinical care, care coordination interventions and evidence-based nursing outcomes. She lectures on topics related to health care delivery models of care, care coordination and cost-effective outpatient care management programs. With passion and compassion, she continues to collaborate with multiple care coordination teams throughout many hospital organizations and Village MD  markets with the goals of successful development and implementation of post-acute care delivery systems. She currently serves on multiple system committee’s focused on improving care coordination and population health of Village MD patients. Nationally, she is a member of several nursing organizations and is currently serving as the American Academy of Ambulatory Care Nursing (AAACN) Chair of the Care Coordination Transition Management Education Evaluation Task Force. The former Chair of the Care Coordination Transition Management Special Interest Group.

Misty received her MBA from Mercer University and a Dual Masters of Science in Nursing as an Adult Nurse Practitioner and Clinical Nurse Specialist degree from Georgia State University, and  her bachelors of nursing degree from Southeastern Louisiana State University, Hammond, LA. She is certified in care coordination transition management (CCTM) by the American Academy of Ambulatory Nursing (AAACN).
